New, revised version of Civil Cover Sheet now available

The new version of the Civil Cover Sheet can be used immediately, and must be used as of 1/6/25. It is available for download here and on the court’s Forms webpage. Questions about the revised form may be directed to media@cand.uscourts.gov.
